Alpine Rehab is a Colorado-based, physician-led rehabilitation medicine group providing in-building physiatry services across SNF/LTC and post-acute settings.
Our work centers on pain management oversight, functional recovery planning, and clear documentation—delivered with consistency and local presence.
We provide focused rehabilitation medicine oversight integrated into each facility’s existing medical structure, strengthening clinical depth while reducing administrative friction.
Our model is intentionally designed to remain physician-led. The physiatrist rounds in the building, providing visible clinical leadership and direct accountability. When NPs or PAs are involved, they collaborate with a Colorado-based physiatrist who is physically present on-site and actively engaged in patient management.
Our model is supported by physician-designed tools and standardized workflows designed to reduce administrative burden while maintaining clear, high-quality documentation.
