For Facility Leadership

Alpine Rehab partners directly with administrators, Directors of Nursing, therapy leadership, and medical directors to provide reliable, regulatory-aware physiatry coverage across skilled nursing and post-acute settings.

Serving skilled nursing and post-acute facilities across Northern and Central Colorado.

How Alpine Rehab supports your team

Alpine Rehab provides structured, evidence-based physiatry services in skilled nursing and post-acute settings. Our focus is functional recovery, regulatory-aware documentation, and safe prescribing practices—designed around your facility’s needs.

  • No facility cost for physician services (billed independently)
  • Billing is distinct from facility medical and therapy providers; may bill same-day as primary care without conflict
  • Documentation directly in the facility EMR to support continuity and survey readiness
  • Rounding schedule aligned to facility preference and census
  • Fall-risk reduction strategies and mobility-focused planning
  • Risk reduction supported by clear clinical documentation and audit-ready records
  • When requested: participation in Medicare/insurance, weekly, and monthly meetings
  • Helps reduce PCP administrative burden (controlled-substance renewals when appropriate, prior authorizations, and other time-intensive paperwork)
  • Staying current on therapies, medications, and tools that improve function and independent living
  • Structured documentation workflows supported by modern clinical systems

Our model supports

Local, physician-led coverage: Our physiatrist rounds in the building. When Advanced Practice Providers (NPs/PAs) are involved, they work in direct collaboration with a Colorado-based physiatrist who is physically present on-site and actively participates in patient care.

Facilities can expect consistent on-site physician presence and real-time clinical availability.

  • Consistent on-site presence and structured rounding
  • Regulatory-aligned pain management oversight
  • Safe prescribing and risk mitigation strategies
  • Reduced avoidable hospital transfers
  • Interdisciplinary care coordination
  • Documentation that supports survey readiness
  • Functional recovery and discharge optimization

Coverage is tailored to your facility’s needs—consistent rounding, timely consults, and clear documentation.
